CommonWell - The project

CommonWell stands for "Common Platform services for Ageing Well in Europe", and is a three year which started in October 2008, spanning 5 countries and 10 partners. It is co-funded by the European Commission within the CIP ICT Policy Support Programme (Grant agreement no. 225005) and will result in 400 users across four locations in Europe receiving newly integrated services for 14 months. The general goal of the CommonWell project is to support independent living for older people and those with long term conditions, integrating and evaluating healthcare and social care services, thus significantly improving the quality of life for individuals and their families.

CommonWell aims to overcome the gaps that separate health and social care service provision in many Member States of the European Union and elsewhere. It delivers ICT-supported, integrated social and health care services at four sites in Europe: Andalucia in Spain, Bielefeld in Germany, Veldhoven in the Netherlands and Milton Keynes in the United Kingdom. At two sites (Andalucía and Bielefeld), platforms and services focus mainly on supporting independent living of older people, while Veldhoven and Milton Keynes focus on the management of prevalent chronic diseases.

To this end, both client-facing and inter-organisational services are developed in close co-operation with older end users and professionals. The services are tested over a 14 months pilot at the four sites.
