CommonWell for patients and other citizens

Growing older, many people need increasing support from healthcare and social care services. For example for carrying out daily living activities such as shopping and personal hygiene, or for managing chronic diseases such as chronic heart failure and chronic pulmonary diseases.

But today social care and healthcare services are often delivered independently by different providers. For older people this can cause practical difficulties like having to run from one provider to another or being unsure about who is responsible for what. As a consequence, the quality of the care services and the quality of life of older people themselves, their family and close relations can suffer.

CommonWell delivers ICT-based integrated services in four countries to demonstrate how things can be done better.

For which people is CommonWell of interest?

CommonWell is of interest to older people that are in need of social care and healthcare services and to their family members and close relations, as well as for older peoples' and patients' organizations.

  • Better quality of social and health care services
  • A feeling of increased comfort and security for older people, their family and close relations
  • Reduction of the number of hospital admissions and of the time spend in hospital for chronic disease patients
  • Improved early intervention and prevention
